| SCHEDULING INFORMATION |

| Our office hours are from 7:30 a.m. - 6:00 p.m. Monday, Tuesday, and Thursday, and 8:00 a.m. - 5:00 p.m. Wednesday. Due to demands of placing of the orthodontic braces, these appointments are usually made in the morning. Also, any appointment that would take more than 30 minutes is scheduled in the morning. |

| We ask that afternoon appointments be alternated between school and after school so that all patients will have an equal opportunity to the after school appointments. It is impossible for us to see all patients after school. We are well aware of the schools policy and we do have excuse forms available for our patients. |

| Extending treatment out over long periods of time by demanding after school appointments causes further possibility of gum problems or decay and will delay the removal of the braces in a timely, efficient manner. |
